The Quality Care Manager is responsible for reviewing patient assessments to ensure appropriate and effective care is provided to each patient in compliance with agency policies, procedures, and regulatory requirements. Trends or patterns of inaccuracy identified by the QCM will be communicated to the Quality Director to assist in the development of the performance improvement and education plan.

Overview
-Must have a California RN license (in good standing)
-Home Health experience and knowledge of Home Care Home Base software a plus.
-Attention to detail and good communication skills required.
-Responsible for processing clinician's visit charting documentation timely.
-Must know home health rules and regulations.
-Full-time remote, work from home position.
-Monday-Friday 8 am to 5 pm.
Responsibilities
Reviews electronically submitted documentation and provide feedback to clinicians regarding deficiencies or OASIS corrections to ensure the medical record reflects compliance with skilled need, medical necessity, and homebound status if required
Ensures visit utilization is supported by individual patient assessment
Reviews and approves final Plan of Care (485) within HCHB
Requirements
Must be a graduate of an accredited School of Nursing
Current unencumbered license to practice as a Registered Nurse in CA
Minimum of 1 year of home health experience with a working knowledge of Title XX11, Medicare Home Health Conditions of Participation, and OASIS
Excellent written and oral communication skills required
Ability to handle stressful situations/deadlines.
Strong computer and software skills with knowledge of Home Care Home Base a plus.
