We are a licensed and growing home health and private duty nursing agency seeking a Clinical Consultant / Subject Matter Expert (SME) to support internal projects aimed at improving clinical documentation, compliance, workflows, and operational efficiency.
This role is ideal for a seasoned professional with deep knowledge of home health regulations, nursing documentation, and day-to-day agency operations. You will collaborate with our leadership and quality assurance teams to optimize processes, ensure compliance with CMS and state requirements, and advise on internal systems and tools.
Key Responsibilities
Provide expert guidance on clinical workflows, forms, and best practices for RN, LPN, HHA, and therapy staff.
Review and help improve documentation standards (OASIS, Plan of Care, Visit Notes, etc.).
Ensure compliance with Medicare/Medicaid, state regulations, and accreditation standards (CHAP, ACHC, or JCAHO).
Advise on internal tools, templates, and process improvements to support efficient and compliant care delivery.
Assist in training development for clinical and administrative staff on documentation and regulatory practices.
Participate in internal audits and quality assurance reviews to identify gaps or areas for improvement.
Qualifications:
Registered Nurse (RN) with current or previous licensure strongly preferred.
Minimum 5 years of home health care experience, including field visits and case management.
In-depth knowledge of OASIS, POC development, care coordination, and CMS home health guidelines.
Experience with home health EMRs and understanding of clinical workflows and documentation standards.
Excellent communication and collaboration skills.
Prior experience in consulting, QA, or clinical operations support is a plus.
Flexible, remote role with part-time hours.
