Overview of the Role: 

We are seeking a Care Specialist to join our digital clinic and help members achieve healthier, more fulfilling lives. As the first point of contact, the Care Specialist plays a vital role in members' behavior change journeys by creating a welcoming onboarding experience and providing ongoing support through recovery management check-ins. This role involves conducting initial screenings/assessments, offering essential resources and referrals, and meeting with members regularly for long-term support. The ideal candidate will demonstrate strong clinical screening, brief intervention, and referral to treatment (SBIRT) expertise, adaptability, and passion for advancing substance use management solutions.

This is a remote position based anywhere in the USA.

In this role you will…

  • Conduct initial screenings and intake assessments to evaluate and collect behavioral health history and symptoms.
  • Understand all Pelago care pathways, effectively communicate treatment options to members, and guide them toward appropriate levels of care.
  • Facilitate warm transitions by connecting members with their state-assigned counselor or coach, sharing relevant background information between both parties based on the member's needs and preferences.
  • Conduct regular check-ins with members (every 3–6 months) through video or phone calls during their care journey to build rapport, enhance engagement, strengthen readiness for change, and enrich their overall care experience.
  • Maintain regular check-ins with members (every 3–6 months) through video or phone calls after program completion to conduct substance use disorder (SUD) screenings, provide early intervention when necessary, and help members reconnect with services if needed.
  • Document member interactions thoroughly and contribute to enhancing the recovery management check-in program.
  • Manage a reduced caseload of members by delivering individual counseling and monitoring progress toward behavior change goals.
  • Utilize advanced evidence-based approaches including CBT, Motivational Interviewing (MI), mindfulness-based interventions, harm reduction techniques, and relapse prevention.
  • Collaborate with cross-functional teams (including product, clinical, and operations) to optimize the member experience and inform service improvements.
  • Maintain up-to-date availability; we’re looking for a commitment of 1 hour per day and at least 20 hours per month.
  • Maintain active licensure(s)/certification(s) in good standing and ensure compliance with all applicable regulations.

The background we are looking for... 

  • Hold an active and unrestricted state credential (license/certification), issued via a state credentialing board.
  • Multi-state licensure is highly preferred.
  • Credentialing recognized by IC&RC or NAADAC is required (e.g., AADC, CAADC, LADC, LCSW with SUD treatment scope, etc.).
  • Minimum of 4 years of clinical experience in substance use disorder (SUD) treatment, with demonstrated expertise in CBT, MI, and relapse prevention.
  • Bachelor's degree in a relevant field.

Additional Qualifications

  • Experience providing SBIRT care for people with SUDs.
  • Excellent attention to detail and strong documentation skills.
  • Experience in program development and quality assurance in clinical settings.
  • Strong communication and organizational skills, with a track record of managing competing priorities.
  • Technologically proficient with experience leveraging virtual tools for care delivery.
  • Bonus: Experience with research, data analysis, or program evaluation.
  • Reliable home office setup with 50–100 Mbps internet bandwidth and a confidential workspace.

The provided range reflects our US target hourly range for this part-time position. Individual pay within the range will vary based on a variety of factors like role-related experience and education, internal pay equity, and other relevant business factors.
