The Opportunity

Aeroflow Health is dedicated to improving health outcomes by addressing Social Determinants of Health (SDoH) and ensuring patients have access to the benefits and resources they need. Our new SDoH program connects patients with essential services, improving their overall well-being and reducing barriers to care.

We are seeking an experienced Community Health Worker (CHW) to support the development and implementation of our new SDoH program. This role (10-20 hours per week to start) involves direct patient outreach, resource navigation, and program development support. The ideal candidate will have hands-on experience in community health work and the ability to provide strategic insights to help shape the program.

 

Your Primary Responsibilities

Program Development & Strategy

  • Lead the design and roadmap of the SDoH program, ensuring its alignment with the goals of improving patient access to essential services through Medicare/Medicaid programs.
  • Collaborate with Aeroflow leadership to define the program’s structure, workflows, and objectives.
  • Establish accountability measures for the program, ensuring the seamless delivery of services.
  • Develop training and resources to support the future team of case workers.
  • Identify and build partnerships with local and national organizations to expand and enhance patient support.

Patient Support & Care Coordination

  • Review patient screening responses to identify SDoH factors impacting care.
  • Coordinate care and assist patients in accessing healthcare services they may be eligible for, including assistance with Medicare/Medicaid enrollment.
  • Manage patient progress by regularly reassessing their care needs and providing ongoing support.

Resource Navigation & Education

  • Compile and direct patients to appropriate resources for addressing identified SDoH challenges.
  • Educate patients on nutrition, chronic condition management, and other topics impacting their health and well-being.
  • Serve as a patient advocate, ensuring they are fully supported in navigating healthcare and social service systems.

Documentation & Data Management

  • Track patient interactions and progress in the electronic medical record system.
  • Monitor patient referrals and outcomes, providing feedback on program effectiveness and areas for improvement.

 

General Responsibilities

  • Employee has an individual responsibility for knowledge of and compliance with laws, regulations, and policies.

  • Maintain HIPAA/patient confidentiality
  • Regular and reliable attendance as assigned by your schedule
  • Other job duties assigned

 

Required Qualifications

  • Minimum of 3-5 years of experience as a Community Health Worker, Patient Navigator, Case Manager, or similar role with a focus on program development.
  • Proven experience building and managing health-related programs, including building strategic partnerships, design, implementation, and evaluation of services along with ongoing process improvement.
  • In-depth knowledge of SDoH factors, public benefits programs, and community resources, particularly in relation to Medicare/Medicaid.
  • Strong case management skills, with the ability to create clear roadmaps, timelines, and accountability measures for program success.
  • Compassionate with excellent communication, problem-solving, and cultural competency skills.
  • CHW certification required.
  • Comfortable using electronic medical records and digital tools for patient tracking and communication.
  • Ability to work independently and manage multiple aspects of a growing program.
  • Must have the flexibility to work the majority of hours during standard business hours to accommodate meetings and program collaboration.

Additional Details

Part-time role (20 hours per week) with an expectation that the majority of hours are worked during business hours.
The goal is for this role to transition to a full-time position within 6-9 months.
