Join our dynamic team at Compass Health Network as a Medical Assistant/Nurse LPN, where your contributions will be vital in delivering compassionate care to the communities we serve. In recognition of the critical service needs, we offer a \(3000 sign-on bonus for Medical Assistants and an additional \)3000 for Nurses. Your role will involve working within a person-centered integrated care team, maintaining professional relationships with patients, their representatives, and medical staff while ensuring adherence to clinic policies and patient confidentiality.
Key Responsibilities
Escort patients from the waiting room and complete vital intake procedures, including medication reconciliation and medical history documentation in the EMR.
Provide end-of-visit materials to patients, including lab results, referrals, and educational resources.
Conduct lab testing as per clinician orders; this may include immunizations, venipunctures, EKGs, and assisting in examinations and procedures.
Review patient-related messages with clinicians and return calls to patients and pharmacies as directed.
Support assigned clinicians with case management, including lab results and medication refills.
Administer injections and maintain VFC log sheets as directed.
Conduct patient interviews to gather necessary data during appointments.
Carry out other miscellaneous duties as assigned by the Supervisor.
Comprehend NCQA Patient Center Medical Home Standards and prepare patient information for the Care Team during morning meetings.
Required Skills
Proficiency in medical office terminology, procedures, and equipment is preferred.
Familiarity with Microsoft Office Suite is advantageous.
Strong communication and interpersonal skills are essential.
Qualifications
High School diploma or GED is required.
A minimum of one (1) year of experience is preferred, although new graduates will receive training.
A Practical Nursing certificate is mandatory for Nurse LPN applicants.
