The NUHS@Home nurse provides nursing care to patients who require acute medical treatment but who are able to be cared for in their homes rather than in a hospital. The nurse works in collaboration with physicians and other healthcare professionals to develop and implement care plans, monitor patient progress, and educate patients and their families on self-care techniques.
The NUHS@Home nurse is competent in performing care assessment and planning, and in the management of patients with identified care needs. S/He provides clinical supervision, precepts the junior nurses and contributes to NUHS@Home nursing training.
Job Responsibilities
Primary Responsibilities and Duties (80%)
Conduct patient assessments and develop individualized care plans
Administer medications, treatments and procedures as ordered by the physician
Monitor and evaluate patient progress and adjust care plans as needed
Communicate with physicians, other healthcare professionals, patients and families regarding patient care
Educate patients and families on self-care techniques and disease management
Ensure patient safety and infection control measures are maintained
Document patient care and progress in electronic medical records
Participate in quality improvement initiatives
Adhere to ethical and legal standards of nursing practice
Utilise technology in care delivery
Secondary Responsibilities and Duties (20%)
Participates in research, clinical or service quality activities
Demonstrates awareness of current nursing and health care practices
Assumes additional responsibilities as assigned by Nursing Officers, Senior Nursing Officers, Assistant/Deputy Director (Nursing) and Chief Nurse
Job Specification/Requirements
Possess Diploma / Degree in Nursing and must be registered with Singapore Nursing Board
Registered Nurse with minimum of 3-5 years of post-registration clinical experience
Ability to work independently and as a team
Strong organisational, coordination, interpersonal and communication skills
Ability to work a flexible schedule, including evenings, weekends and public holidays
