What is the Health Navigator Deployment Specialist role?

The Remote Health Navigator Deployment Specialist shall monitor, receive and screen incoming alerts, calls and chat communications. They will schedule and participate in audio-visual calls with patients who are enrolled in our remote monitoring program. They’ll work closely with a Registered Nurse and report to them any abnormal values or other patient concerns. The Health Navigator acts as a liaison for patients to ensure that all needs are met during monitoring program enrollment. They shall work with all members of the clinical care team to enhance health care outcomes and take the initiative to ensure clinical care and related support services are made available to patients. They shall think critically and creatively about what is needed to provide virtual in-home support while assisting with the low-level tech support and troubleshooting for patients.

The Health Navigator Deployment Specialist will also service local participants in the Syracuse area by deploying devices to their homes upon enrollment.They shall ensure that all participants are educated on how to utilize and maintain their monitoring devices during enrollments.

This is a Full-Time position. Schedule: 4 Weekdays + Saturdays (7a-3p)

What you’ll be doing:

  • Monitor all alerts.
  • Answer, direct, and follow up on incoming calls.
  • Review and maintain follow up tasks.
  • Receive and perform virtual follow up calls and document all information obtained on the call in the appropriate charting systems.
  • Support participants' physical and emotional wellness with a holistic health approach.
  • Addresses social determinants of health by supporting participants through loneliness, anxiety, and depression.
  • Communicate concerns, changes in condition and all medical complaints to the Registered Nurse.
  • Troubleshoot issues with technology and escalate technical difficulties that cannot be resolved.
  • Schedule, manage and complete participant home visits to deploy devices for new patients and provide in-home education and troubleshooting as needed.
  • Other duties as assigned.

What you’ll bring:

  • High school diploma or GED required.
  • Must have a valid NYS drivers license and reliable vehicle.
  • Up to 25% Travel within Syracuse, NY area required.
  • Local to Syracuse, NY preferred
  • Strong computer skills and ability to learn and use new technology.
  • Empathy, compassion, and patience.
  • Superior listening, verbal, and written communication skills.
  • High level of professionalism and ability to work independently and with a team remotely.
  • Strong problem-solving abilities with attention to detail.
  • Ability to handle stressful situations appropriately.

  • Must be able to work from home with a HIPAA compliant environment.
  • Must be flexible to work some evenings, holidays and weekends.
